New Patients

We welcome new patients to register at Station House Surgery who live within our practice area. Once registered you will be allocated a Usual GP.  However, if you have a preference as to whom you are registered with, please speak to a member of our reception team. 

All new patients that register at Station House Surgery will be asked to complete a new patient registration form and new patient health questionnaire (links to both can be found below).

 

Please complete and submit both forms online:

  • Registration form (GMS1)
  • New Patient Questionnaire (16+ OR under 16 years old)

 

Alternatively you can print the forms or ask for the forms at the reception desk.

Temporary Patient Registrations

 

Should you become unwell whilst away from home, or be residing in the GP Practice catchment area for up to three months, then you can register as a temporary patient at Station House Surgery.

However, given the use by most GP practices of remote consultations you may still be able to access treatment from your GP practice whilst you are away from home.
