If you have a Long Term Condition such as: Respiratory (COPD or Asthma), Vascular (Hypertension, Atrial Fibrillation, Coronary Heart Disease, Cardiovascular disease, Heart Failure, Peripheral Arterial Disease, Stroke) Diabetes, High Blood Pressure, then you will need to have your condition monitored regularly.
We will contact you by either text, letter or phone and invite you to book a review appointment.
Appointments can be booked via the link in your text message, on-line (using the NHS App or Patient Access) or by speaking to the Reception Team who will be happy to help.
We have recently introduced a new system for recalling our patients with long-term conditions during their month of birth. This means that we will invite you annually during the month you were born and hopefully it will mean that you may need to attend for less appointments. This may vary from patient to patient depending on when you last attended for a review but we feel it will be helpful as it may mean you do not have to attend for as many appointments. If you have received an invite for your annual review during your birth month but, have already had your annual review in the last 3 months, you do not need to attend on this occasion and you will be contacted when your review is next due.
